Resumo: Syphilis is a sexually transmitted disease caused by Treponema pallidum. Its clinical manifestation presents different stages of evolution, presenting distinct oral lesions in each one of them. A 38-year-old male patient was referred for evaluation of ulcerated lesions on the lateral border of the tongue and fornix of the anterior vestibule, with 40 days of evolution. The patient reported that in addition to the oral lesions, he had weight loss, fever and fatigue. An incisional biopsy of the lesion located in the anterior vestibule region was performed and histopathological analysis revealed non-specific chronic ulceration. The patient was referred to the center for infectious and contagious diseases, being diagnosed with syphilis and HIV/AIDS, remaining hospitalized for 6 months. The patient was treated and currently he is well with an undetectable viral count in the blood. Accurate diagnosis and correct referral are very important to determine the appropriate treatment strategy.
