Resumo: Oral soft tissue necrosis is a rare complication of head and neck radiotherapy that, although poorly recognized in the literature, is associated with significant morbidity in cancer survivors. We report here a case series of 4 patients with a mean age of 60 years at diagnosis. The medical history of the patients included squamous cell carcinoma of the posterior regions of the oral cavity, diabetes mellitus, hypertension, intracranial aneurysms, and risk of thrombosis. The clinical diagnosis of soft tissue necrosis was established between 2 and 5 months after radiotherapy with clinical presentations in the form of extensive chronic ulcers and painful necrotic masses. Patients were treated using protocols of oral hygiene, photobiomodulation and PENTO. The response to treatment, as reported in the literature, is slow but the patients are permanently under follow-up and support. The diagnostic and therapeutic challenges of this oral toxicity from radiation therapy will be discussed.
