Resumo: Hyperplasia of the sublingual gland is a common condition that presents as an enlargement of the floor of the mouth in elderly patients who are fully or partially edentulous. However, it may be clinically misdiagnosed, leading to unnecessary biopsies and concerns. Therefore, we report four cases of this condition. All patients presented with swelling in the floor of the mouth, and clinical diagnoses of inflammatory fibrous hyperplasia (two cases), lipoma, irritation fibroma and salivary gland neoplasia were suggested. Three patients were female, and their ages ranged from 55 to 91 years. Excisional biopsy was performed in all cases, revealing lobules of the salivary gland containing normal serous, mucous, seromucous acini, acinar atrophy, fibrosis, and substitution by adipose cells. Clinicians should be aware of this condition, which does not require intervention unless necessary for better prosthesis fitting.
