Resumo: Ameloblastic fibro-odontoma (AFO) is a rare, asymptomatic, benign, mixed odontogenic tumor. It affects a population between the 1st-2nd decades of life, most frequently in the mandible. Surgical removal is conservative, and the prognosis is favorable. We report a case of AFO in an 8-year-old male who sought dental care for an asymptomatic lesion surrounding the tooth 47, with delayed eruption. Imaging examinations showed well-defined radiolucent lesion with small calcifications around the crown of element 47. On examination, a rounded, normochromic, irregular, firm lesion measuring 25 mm was observed in the region of the right retromolar triangle. The diagnostic hypotheses of calcifying odontogenic cyst and dentigerous cyst were raised. An aspiration puncture was performed, without evident liquid content, followed by an excisional biopsy of the fibrous capsule, with maintenance of the tooth, whose histopathological diagnosis was AFO. The patient has been followed up for six months without evidence of recurrence.
