Resumo: Pseudoperineurioma (PSPN) is a recently described reactive lesion characterized by concentric perineural proliferation around axons and Schwann cells commonly diagnosed as traumatic neuroma (NT). Objective: To reevaluate the clinicopathological aspects of NT, perineuriomas (PN) and PSPN diagnosed in 6 Oral Pathology laboratories. Study Design: Clinical data were obtained from the requisitions, and slides were reevaluated. Chi-square test (p<0.05) was used. Results: After reclassification, 152 (65.51%) NT, 48 (20.68%) PSPN and 1 PN (0.43%) were found. NT and PSPN affected more women, white adults, presented as nodules with a hypothesis of fibrous lesion. While PSPN occurred in the tongue (77.08%), NT occurred more in the lip (28.94%), tongue (21.81%) and mental foramen (20.39%). PSPN were more superficial, associated with fibrous tissue and less inflamed than NT. Among PSPN, 62.5% presented nerve tissue involving up to half of the lesion. NT characterized by nerve fascicles cut in different directions delimited by perineurium had a higher percentage of symptoms than those characterized by diffuse microfascicles within fibrosis. Conclusion: This is the largest series of NT/PSPN cases ever evaluated. Despite the similar features, the pathologist should be aware of perineural proliferation in PSPN, not to mistake it for NT (reactive lesion) or perineurioma (neoplasm).
