Resumo: Peripheral odontogenic keratocyst (POKC) is a rare soft tissue lesion that has the same histological characteristics as odontogenic keratocyst. Here we report a rare case of peripheral odontogenic keratocyst affecting the gingiva in a 56-year-old woman. The patient had no relevant medical history. Clinically, a painless swelling was observed in the gingiva between teeth 43 and 44. Microscopically, the lesion showed classic characteristics of intraosseous odontogenic keratocyst such as a palisade basal layer, stratified squamous epithelium containing 4 to 8 layers and a superficial layer rich in parakeratin. Conservative surgical removal was performed, and we have not observed recurrence at the time of this publication. Peripheral odontogenic keratocyst is a rare lesion that mainly affects older adults. In addition, they can be confused with other odontogenic lesions or salivary gland neoplasms, depending on the lesion, in the present case the diagnostic hypothesis was proliferative inflammatory process due the location.
