Resumo: A 37-year-old male patient with a history of herpes reports multiple lesions nine days ago with rupture of blisters, yellowish red ulcer-like lesion, sharp boundaries, irregular shape, Nikolsky sign positive, induced blisters, spontaneous pain, bleeding to the touch and with partial displacement of the mucosa at digital pressure. Complementary tests showed IGM negative and IGG positive for Herpes virus; IGG and IGM negative for FTA-ABS; VDRL negative and HBSAg non-reagent. At microscopy was observed fragment of oral mucosa lined by parakeratinized stratified paving epithelium exhibiting acanthosis and spongiosis with areas of discontinuity of the epithelium. In dense fibrous connective tissue, blood vessels and the presence of perivascular lymphocytes are observed. The histopathological diagnosis of chronic inflammatory process was made and the management with acyclovir and Prednisolone. After medication, the patient presented improvement of the clinical appearence and symptomatology as also he has been into five months follow-up.
