Resumo: A 65-year-old male patient, leukoderma, reports two lesions three months ago, on the right jugal mucosa and right upper lip without significant improvement in healing after the use of Ad-Muc® - Chamomilla recutita (L) fluid extract as well as plaques on the skin. At clinical examination, a lesion was observed in the upper labial mucosa comprising reddish ulcer, single, clear boundaries, oval shaped, non-infiltrative, sensitive to touch, with ulcerated surface and measuring 1.5 cm. At microscopy, a fragment of an immune-mediated lesion characterized by the presence of a suprabasal cleft containing acanthotolytic Tzanck cells was found. The surface epithelium is stratified parakeratinized pavement and the connective tissue is of the dense fibrous type containing mild chronic inflammatory infiltrate. The histopathological diagnosis of pemphigus vulgaris was given. The management was the administration of prednisolone and the patient is under ten months of follow-up and discontinuation of the use of corticosteroids.
