Resumo: A 16-year-old female patient diagnosed with Lennox Gastaut syndrome was admited at the intensive care unit due to lowered level of consciousness and respiratory failure. Atropine was administred to control sialorrhea. However an important tongue enlargement was noticed after the use of medication, diagnosed as macroglossia secondary to angioedema. Patient had involutary and unconsciousness jaw moviment which caused an acute traumatic injury and she lost a 2cm fragment of the tongue. Corticosteroids were prescribed, daily intraoral photobiomodulation (660nm, 100mW, 1J/point, 33J/cm2) was performed and an intraoral device to protect biting and damage was positioned. Angioedema regressed ten days after atropine withdrawal, tongue healing was achieved, however a permanent injury to tongue occured. Massive macroglossia is a rare complication of angioedema, especially associated to atropine.
