Resumo: Oral verruciform xanthoma (OVX) is an uncommon benign lesion of unknown etiology that resemble other papillary lesions. A 26-year-old male with a history of hematopoietic cell transplantation for treatment of acute myeloid leukemia, reported the appearance of an asymptomatic lesion with 5 months of evolution. Clinically, a sessile verrucous lesion with a slightly yellowish appearance and 1 cm in diameter was observed, located in the alveolar ridge. The differential diagnosis included squamous papilloma and verrucous carcinoma, for which an incisional biopsy was performed. Microscopically, it exhibited papillary epithelial hyperplasia forming uniformly elongated bulbous ridges, under which numerous xanthomized cells confined to the conjunctival papillae can be observed. The CD68+ immunohistochemical reaction showed xanthomized macrophages, allowing the histopathological diagnosis of OVX. Subsequently, an excisional biopsy was performed, and the patient is currently free of the lesion. The histopathology and clinical history are highlighted as auxiliary methods in the diagnosis of OVX.
