Título: Clinical fluorescence as a diagnostic and prognostic tool for assessing potentially malignant oral disorders

Resumo: Objective: Clinical fluorescence (VelScope) has been utilized as an adjuvant
tool in some clinical conditions, mainly to select a proper area for biopsy. Our
purpose in this pilot study was to assess the usefulness of this device in
predicting epithelial dysplasia levels in potentially malignant conditions, mainly
leukoplakia and actinic cheilitis (AC).
Study design: The patients enrolled in the study were selected from our
institution's outpatient clinic. All patients with a clinical suspicion of leukoplakia
or AC underwent VelScope examination, followed by biopsy. Histological results
were compared with fluorescence features.
Results: Eleven patients were diagnosed with leukoplakia, comprising seven
women and four men with a mean age of 59.6 years, and 15 with AC,
comprising four women and 11 men with a mean age of 61.4 years. Epithelial
dysplasia grades and the loss of fluorescence levels were concordant in 54.5%
of leukoplakia cases and 60% of AC cases; high discrepancies were noted in
18.1% of leukoplakia and 6.6% of AC cases.
Conclusion: Fluorescence evaluation method is helpful in predicting the level of
epithelial dysplasia in cases of potentially malignant disorders, showing a
significant level of conformity between the clinical assessment and microscopic
study of such cases.
