Resumo: Spindle cell lipomas are extremely rare in the oral cavity, especially in the hard palate, and can be mistaken for liposarcomas due to their similar histological features. We present a case of a 53-year-old male that was referred with an asymptomatic lesion on the hard palate with 35 years of duration. Intraoral examination showed a pedicled nodule with soft consistency in the midline region of the hard palate, covered by normochromic and smooth surfaced mucosa, measuring approximately 3x3cm. Patient's medical history was not relevant. Main diagnostic hypotheses included a benign tumor from salivary gland or mesenchymal origin. Since there was no suspected malignancy or signs of local invasion, an excisional biopsy was performed. Microscopic examination provided a preliminary diagnosis of spindle cell lesion. Upon immunohistochemical study and combining clinical findings, the final diagnosis was Spindle Cell Lipoma. The patient is still being monitored and shows no signs of recurrence.
