Resumo: Histoplasmosis is a deep fungal infection and its oral manifestations are often associated with disseminated disease. A 51-year-old female was referred to the Oral Medicine Service reporting pain on her tongue, weight loss and eating difficulties with a 2-month evolution. Her medical history included HIV+, hypertension, and asthma. However, she was not on antiretroviral therapy. She recently quit smoking and reported daily beer consumption (about 2.8L). Intraoral examination revealed white pseudomembranous plaques removable by scraping scattered over the bilateral buccal mucosa and a single deep ulcerated lesion with raised edges, measuring approximately 1.5cmx0.5cm, covered by a fibrinopurulent membrane, on the tongue dorsum. An incisional biopsy was performed and the histopathological analysis showed chronic granulomatous inflammation. Grocott-Gomori stain showed positive round structures, suggesting histoplasmosis infection. Afterwards, she was treated with 400mg itraconazole daily by the infectology team.
