Resumo: Central odontogenic fibroma (COF) is defined as a benign neoplasm characterized by presence of an inactive odontogenic epithelium within a fibrous connective tissue stroma. A 42-year-old male patient was referred to the oral surgery service after experiencing pain in the mandible. Clinical examination, no intraoral or extraoral alterations were observed. Imaging exams revealed a well-circumscribed radiolucent area in the right mandible associated with tooth 48, which was positioned horizontally and in close contact with the mandibular canal with no extension to the cementoenamel junction. The treatment consisted of lesion excision, and the removal of tooth 48, which was completed without complications. The histological, morphological, and imaging aspects confirmed the diagnosis of COF. The patient has been under clinical and radiographic follow-up for 12 months, with no complaints, signs of local bone formation and no recurrence. COF is a rare lesion and the correct diagnosis is important for adequate treatment.
