Resumo: Angiomyolipoma (AML) is a common renal benign neoplasm with malignant transformation potential. Extrarenal manifestations are rare but sarcomatous transformations have been reported in sites such as lungs, liver and in the head and neck. We report a case affecting a 38-year-old male patient, referred for evaluation of a floating subcutaneous nodular lesion in the lateral region of the face. An excisional biopsy was panned with clinical hypothesis of benign salivary gland neoplasm, or another benign soft tissue tumor such as lipoma. After a histopathological report of Angiomyolipoma with sarcomatous transformation it was investigated through immunohistochemistry the following markers: CD31, CDK4, HMB45, Calponin, MDM2 and Ki-67, which concluded the diagnosis of High Grade Angiomyolipoma with sarcomatous transformation. It was performed a CT PET scan of head and neck region, followed by neck dissection and surgical margin enlargement. The patient than started chemotherapy and is already under follow up for seven month.
