Resumo: Idiopathic gingival fibromatosis is a rare non-inflammatory gingival hyperplastic lesion, with a slow and progressive course, which affects quality of life in functional, aesthetic and psychological areas. Female patient, 31 years old, complained of an asymptomatic exophytic lesion in the region of the palate that appeared in childhood and worsened in adolescence. On physical examination, two extensive pedunculated nodules were observed on the hard palate, bilaterally, originating from the gingiva attached to the palate of the maxillary teeth. Upon palpation, firm consistency and smooth surface were observed. The diagnostic hypothesis was idiopathic gingival fibromatosis, due to clinical findings and reported history. An incisional biopsy was performed. The anatomopathological analysis confirmed the diagnosis of gingival fibromatosis. As it was an extensive lesion, the post-diagnosis procedure was surgical excision under general anesthesia. The procedure was uneventful. The patient has a satisfactory evolution and no signs of recurrence.
