Resumo: A 17-year-old female patient complaining of "swelling in the cervical region". Physical examination revealed an immobile tumor mass, with undefined limits, symptomatic with two months of evolution. The diagnostic hypotheses were Lymphoma, Kuttner´s Tumor, Sporotrichosis, Tuberculosis and Cryptococcosis. The patient denies prodromal signs and contact with animals. An ultrasound of the cervical region showed a diffuse hypoechoic area located between the parotid and submandibular glands involving lymph nodes. Blood tests were negative to CMV, anti-HIV 1/2, IGRA and reactive to PPD. An incisional biopsy was performed and a culture test for microorganisms was collected. The anatomopathological analysis revealed a non-specific mixed inflammatory process (necrotizing lymphadenitis) and a positive culture for Mycobacterium tuberculosis, with final diagnosis was ganglionic cervical tuberculosis. Despite the diagnostic challenge, the patient was treated and remains under clinical follow-up.
