Resumo: This work aims to report two cases of oral lesions with suspected malignancy that were infectious diseases. Case 1, a male patient, 75 years old, smoker, presented a single lesion, ulcerated with whitish-yellow plaques and granulomatous areas in the belly of the tongue and right lateral border, measuring 3 cm, local edema, hardening and reduced mobility and asymptomatic. Incisional biopsy and serological examination confirmed the diagnosis of paracoccidioidomycosis. Case 2, a 45-year-old male patient, smoker and alcoholic, had an ulcer in the left labial retrocommissure with raised, hardened edges measuring 2 cm and associated burning. Incisional biopsy confirmed BAAR, and after chest tomography and sputum examination, the diagnosis of tuberculosis was confirmed and after 3 months of treatment presented regression of the oral lesion. The individualized diagnosis of oral lesions needs to include infectious lesions, which can mimic malignant neoplasms and hinder the correct diagnosis, delaying the correct treatment.
