Resumo: INTRODUCTION
Paracoccidioidomycosis (PCM) is a systemic fungal disease that is associated with rural environments and agricultural activities. Though a rare disease worldwide, and PCM is most common endemic deep mycosis in Brazil.

OBJECTIVE
To evaluate the prevalence and clinical-epidemiological profile of patients with oral PCM and treatment adherence.

STUDY DESIGN
Retrospective study of medical records of patients diagnosed with oral PCM who received treatment between 1989 and 2022. The inclusion criteria were the presence of oral mucosa lesions and confirmation of PCM diagnosis through serological or histopathological analysis.

RESULTS
From a total of 14.519 record, 97 patients were diagnosed with PCM (0.6%) and 90.6% of these cases were men. Most individuals engaged rural activities (30.9%) and presented smoking (81.4%) and alcohol (52.7%) habits. All patients had moriform ulcers that mostly affected gingiva (51.5%). Only 18% of patients adhered to the treatment, being most patients treated with itraconazole (28.5%) and remission of lesions was commonly observed after 1 to 3 months (26.8%).

CONCLUSION
Brazil southeast region is an endemic area and the profile of PCM patients included in this study was similar to described in the literature. In addition, although itraconazole treatment have been effective, the adherence to treatment is still an obstacle for this serious disease.
