Título: ORAL LICHEN PLANUS AND ORAL LICHENOID LESION: CHARACTERIZATION OF THE LYMPHOCYTE PROFILE

Resumo: Objective To investigate the profile of the lymphocytic inflammatory infiltrate in oral lichen planus (OLP) and oral lichenoid lesion (OLL). Study design Clinical information and histopathological slides from 72 cases compatible with the clinicopathological diagnosis of LPO and LLO were reviewed, followed by immunohistochemical investigation for anti-CD4 and anti-CD8 antibodies to characterize the lymphocyte profile and submitted to bivariate (x²) analysis. Results Non-smoking, middle-aged white women with asymptomatic white lesions involving multiple sites in the oral cavity were the most affected. Cases with a diagnosis of OLP without epithelial dysplasia, with intense subepithelial lymphocytic infiltrate predominated. There was a significant association between OLP and bilaterality (p=0.001), absence of epithelial dysplasia (p≤0.0001), and paraceratin (p=0.041). All cases were positive for CD4, and there was an association between high CD4 expression with absence of symptoms (p=0.045) and diagnosis of OLL (p=0.045). CD8 positive cases predominated, there was an association between low CD8 expression and absence of epithelial dysplasia (p=0.041). Conclusions Thorough clinical and histopathological evaluation is fundamental for the differential diagnosis between OLP and OLL. The high expression of CD4 correlated with the diagnosis of OLL evidences that the inflammatory infiltrate profile of these lesions may contribute to its diagnosis.
