Resumo: A 59-year-old white male with complaints of recurrent pain in the anterior region of the maxilla underwent dental care. He reported a history of dental trauma in the region of the upper central incisors, which were endodontically treated 30 years ago. On clinical examination a fistula was observed and on imaging exams a radiolucent image of cystic appearance was observed, measuring 2.0x2.5 cm. Periapical cyst, lateral root cyst and nasopalatine duct cyst were the diagnostic hypotheses. An excisional biopsy was performed, the macroscopic image of the lesion had a capsular aspect, like a cyst, however, the histopathological report was compatible with intraosseous squamous cell carcinoma. This case report shows that malignant lesions may present diverse clinical manifestations, which may surprise professionals during the diagnostic process. The manager should be careful in all cases and always send the biopsy tissue for histopathological analysis even in the absence of suspicion of malignancy.
