Resumo: A 57yo male patient with prostate adenocarcinoma Gleason 9(4+5) treated with local radiotherapy and chemotherapy in the period from 2020 to 2022. He complained of a nodule in the mandible within 7 days growing without painful symptoms. He presented a 6mm submucosal nodule in the vestibular region of the mandible, with the same color as the normal mucosa with regular edges, softened on the touch, painless. CBCT showed diffuse bone rarefaction with a "moth-eaten" appearance with multiple fenestrations and cortical discontinuity with rupture of the corticals. The primary hypothesis was metastasis of prostate adenocarcinoma. An incisional biopsy was performed and the diagnostic report showed malignant neoplasm arranged in nests with positive staining for prostatin and PSMA in immunohistochemistry. The final diagnosis was adenocarcinoma metastasis with probable origin in the prostate. The treatment now included mandibular radiation therapy in association with zolendronic acid. The patient is being followed up.
