Resumo: Infectious mononucleosis (IM) is a self-limited infection caused by Epstein-Barr Virus (EBV). A 14-year-old male patient presented to the attendance showing fever, discomfort, and sore throat. In the intraoral exam were observed a painful ulcer on the superior lip and mucosa and a hyperplastic tonsil with whitish exudate. On extraoral exam, the patient showed eyelid edema, bilateral submandibular lymphadenopathy, and erythematous macular rashes in the torso. An incisional biopsy of the ulcerative lesion revealed a proliferation of cells of lymphoid origin, mainly plasma cells, arranged in a follicular pattern. Also was observed the presence of eosinophils and neutrophils. A chemiluminescence exam showed negative IgM and positive IgG antibodies against EBV, indicating previous contact with the virus, thus pointing to the diagnosis of IM. The patient was forwarded to the infectologist, that prescribed a supportive treatment and followed the case until the illness regression. Support: CAPES, CNPq and FAPEMIG.
