Resumo: Hairy leukoplakia presents as a white plaque, usually on the lateral border of the tongue, and is associated with the Epstein-Barr virus. It is common in immunosuppressed HIV patients, but it can also occur in HIV-negative patients with other types of immunosuppression. We present three clinical cases, two of whom are women who were on corticosteroids due to systemic lupus erythematosus and rheumatoid arthritis. All three patients presented bilateral white plaques on their tongues and underwent biopsies, with the initial hypotheses of lichen planus, verrucous leukoplakia, hyperkeratosis, and hairy leukoplakia. Microscopically, hyperparakeratosis was observed, and below the keratin layer, ballooning keratinocytes with nuclear beading and positivity for anti-EBV antibody were observed. It is important for clinicians and pathologists to be aware that hairy leukoplakia is not a disease exclusively associated with AIDS and that investigation for immunosuppression is crucial when diagnosing this condition.
