Resumo: Objective: To describe the clinicopathological aspects of chronic sclerosing sialadenitis of the submandibular gland and those in the spectrum of IgG4-Related Disease (DR-IgG4).Study Design: Focal and diffuse Chronic inflammation, periductal fibrosis and sclerosis, reduction of the secretory parenchyma, lymphoid follicles, and those which are common in DR-IgG4 (obliterative phlebitis, storiform fibrosis, dense lymphoplasmacytic infiltrate, eosinophilic infiltrate, presence of sialolith) were identified). All cases were according to the following scores: absent, mild, moderate, and abundant. Demographic and clinical data were also included. Results: Seventeen cases were identified. The stage 3 was the most frequent, with only three cases in the DR-IgG4 spectrum and 53% had sialoliths. There was a predilection for males, the mean age of 55 years, 3.8 cm on average diameter, equally affecting both sides.Conclusion: Chronic sclerosing sialadenitis is a rare fibro-inflammatory disease with a varied histopathological spectrum, in which DR-IgG4 must be included in the differential diagnosis.
