Resumo: An 11 years-old male presented an asymptomatic increase of volume on the left side of face. In the imaging exams, a radiolucent lesion was identified in the posterior mandible. In the incisional biopsy, the diagnosis of ameloblastic fibrodentinoma was established and surgical removal was performed. Two years later, the patient exhibited recurrence of the lesion and, through a new biopsy, the diagnosis of ameloblastic fibroma (AF) was rendered. However, 15 days later, the lesion exhibited rapid growth and displaced the dental elements 35, 34, and 33 and the imaging exams evidenced destruction of cortical bone. An in-block surgical resection was performed with diagnosis of ameloblastic fibrosarcoma (AFS). At the moment, the patient is undergoing cancer treatment, received chemotherapy and radiotherapy sessions. Patients with an initial diagnosis of AF should undergo periodic follow-up due to the recurrence risk and malignant transformation into AFS, which is an extremely rare malignant odontogenic tumor.
