Título: AMELOBLASTIC CARCINOMA IN THE MANDIBLE: A CASE REPORT

Resumo: A 30-year-old man presented with a large lesion in the posterior region of his mandible. Intraorally, the lesion appeared exophytic, mottled, pedunculated, with a different contour than the adjacent mucosa, and with a previous diagnosis of ameloblastoma. A panoramic radiograph showed a wide, well-defined unilocular radiolucent lesion. 3D imaging from the tomography revealed extensive tissue destruction. Histopathological analysis showed a fragment of oral mucosa covered by stratified squamous epithelium with irregular exophytic projections, presenting hyperchromatism, pleomorphism, and atypical mitoses. The epithelium infiltrated the underlying connective tissue with dyskeratosis, loss of adhesion, and a reticulum-like appearance of the enamel organ, with bizarre cells, large nuclei, and frequent atypical mitoses, sometimes forming aneuploid or mitotic catastrophe figures. There were also more organized areas with a plexiform appearance and a hyperchromatic basal layer arranged in a palisade pattern with nuclei showing inverted polarization and cytoplasmic vacuolization. The conclusion was ameloblastic carcinoma.
