Resumo: Pycnodysostosis is a rare autosomal recessive syndrome, characterized by abnormalities in the bone remodeling process and increased susceptibility to pathological fractures and osteomyelitis. This study aims to report a case of a 49-year-old male patient with pycnodysostosis that complained of an extraoral swelling in the left mandible. Clinical examination revealed a swelling in the left submandibular region, intraoral fistula with purulent drainage, and a history of tooth extraction three months ago. Panoramic radiography showed a pathological fracture in the left mandibular body. The treatment was resection of the infected area, surgical debridement, installation of a mandibular reconstruction plate, and fistulectomy. The diagnostic hypothesis was osteomyelitis, confirmed by excisional biopsy. After 3 weeks, intra-oral wound dehiscence, intra-oral purulent drainage, and extra-oral fistula were observed. Antibiotic therapy was performed and the patient was submitted to curettage, debridement, and fistulectomy. Currently, the follow-up showed complete healing and no recurrence of the infection.
