Resumo: A 78 years-old male with a history of chronic lymphocytic leukemia and autoimmune hemolytic anemia since 2016, was hospitalized in November of 2020 with four days of fever reaching 39ºC. His brackground included type II diabetes mellitus. He did not present signs of cough, urinary infection, skin lesions or gastrointestinal alterations. Heart examination showed mitral regurgitant murmur auscultation. Transesophageal echocardiography confirmed a vegetative and flail lesion with significant regurgitation involvement. Antibiotic therapy was started with ampicillin and ceftriaxone, guided by blood cultures that were positive for Enteroccocus fecallis. Dental team was called to screen for oral infectious foci. Oroscopy showed partial endentulism with four remnants teeth upper incisors, and one residual upper right first premolar. Perimplantitis and periodontitis were also noted. Antibiotic coverage was maintained for periodontopathogenic bacteria and mouthwash with 0.12% chlorhexidine was prescribed. The patient was instructed about dental treatment after hospital discharge and deceased five months later.
