Resumo: Two white-skinned 80 years-old women with no history of smoking or alcohol intake were referred to the Oral Medicine Clinic due to large heterogeneous leukoplakias, at the border of the tongue in one patient and at the anterior mandibular alveolar mucosa in the other. The lesion of the first patient has 4 years´ duration and the second had a previous diagnosis of oral lichen planus. Incisional biopsy of the first case revealed malignant epithelial proliferation diagnosed as squamous cell carcinoma, and of the second resulted in epithelial hyperparakeratosis and hydropic degeneration of basal layer with numerous features of severe dysplasia, which along the clinical data, was diagnosed as proliferative verrucous leukoplakia (PVL). PVL is an aggressive form of oral leukoplakia, however heterogeneous leukoplakia in elderly woman without history of harmful habits also present high risk of malignant transformation demanding regular follow-ups and biopsies to ensure at least an early diagnosis.
