Resumo: A 47 years-old caucasian woman was referred due to multiple brown macular lesions on her gum over one year. Her background included use of mouth washing and hair loss therapy. She denied smoking and alcoholism, and used escitalopram. Her sister and aunt had been diagnosed with pituitary adenoma. The patient underwent a lip biopsy and histopathology diagnosis revealed fixed pigmentary erythema. Based on the diagnostic hypotheses of oral melanoacanthoma and Addison´s disease, the biopsy material was sent for immunohistochemical assessment of the biomarkers AE1/AE3, S-100, HMB45, Melan-A and Ki-67. Serum level of adrenocorticotropic and cortisol hormone were normal. The diagnosis was multifocal oral melanoacanthoma and she has been followed for the past five years with a dentist and an endocrinologist. She has been presenting variation in the location and intensity of the macules. In 2022 a gingival biopsy was performed and the diagnosis of melanoacanthoma was again confirmed.
