Resumo: Oral squamous cell carcinoma (OSCC) is considered a differential diagnosis of subgemal neurogenic plaque (SNP), but the association of both adjacent lesions is extremely rare. A 47-year-old female patient reported severe pain in the posterior third of the tongue. Intra-oral examination revealed multiple reddish rounded papules, that were soft to palpation. Incisional biopsy was performed with a histopathological diagnosis of SNP and lymphoid tissue hyperplasia. Local measures were taken to prevent trauma in the region. However, symptoms did not diminish, and it changed location to the middle third of the tongue. A new erythematous lesion was evident next to where the SNP was removed. A second biopsy was performed with a histopathological diagnosis of OSCC. The patient was referred to oncology services. While SNP usually causes severe pain, it is uncommon in early OSCC, but when associated, pain may be the first sign of malignancy.
