Título: GENERALIZED GINGIVAL ERYTHEMA AND THE DIAGNOSIS OF MUCOUS MEMBRANE PEMPHIGOID: A CASE REPORT

Resumo: A 55-years-old black skinned male patient went to the Oral Diagnosis Clinic complaining of pain and bleeding in the gums. Intraoral examination showed generalized erythema in the attached gingiva and excessive accumulation of dental calculus. To rule out the diagnosis of plaque-induced gingivitis, and to reduce the inflammation prior the incisional biopsy, supragingival scaling was proposed. During this procedure a gingival bubble was unintentionally induced and interpreted as a positive Nikolsky sign. The biopsy demonstrated a subepithelial blistering diagnosed as mucous membrane pemphigoid (MMP). The patient started a therapy with topical corticosteroid, and was referred to ophthalmologist and dermatologist, and remains under follow-up. The Nikolsky´s sign facilitated the investigation and diagnosis of PMM, but in general, intense gingival inflammation and/or that do not respond satisfactorily to periodontal therapy, may constitute a clinical sign of systemic autoimmune and immune-mediated diseases , highlighting the importance of dentists in diagnosing these diseases.
