Resumo: A 21-year-old Caucasian female was referred for evaluation of an asymptomatic expansile lesion in left posterior maxilla of unknown evolution time. Intra oral examination showed a swelling in the left upper alveolar ridge extending to the hard palate, firm on palpation. Imaging exams demonstrated an expansive radiopaque/hyperdense maxillary destructive lesion invading the left maxillary sinus, close to the orbital floor. An incisional biopsy was performed, and microscopic analysis showed spindle cell proliferation in a scanty stroma and small calcified nodules leading to the diagnosis of Psammomatoid Ossifying Fibroma. The patient was submitted to an intra oral local excision, and she was rehabilitated with partial prosthesis. After 2 years, the patient was surgically rehabilitated with iliac crest bone graft for subsequent dental implant placement. However, the bone graft was partially removed due to complications. The patient remains well in regular basis clinical follow-up without evidence of recurrence.
