Resumo: Oral lichen planus (OLP) is an autoimmune condition that affects the squamous epithelium of the oral mucosa and is also described as a potentially malignant disorder. OLP often affects female patients, and it is rarely observed in adolescents and young adults. Hereinafter, we report a case of a 20 years-old patient diagnosed with OLP. In the physical examination, the most important findings were the hyperpigmentation lesions in the lower lip, and the classical presentation of reticular OLP, bilateral symmetrical striae on the buccal mucosa. Under the clinical hypothesis of OLP an incisional biopsy was performed. Microscopically, areas of atrophic epithelium with saw-tooth rete pegs, liquefactive degeneration of the basal cell, Civatte bodies, and a dense band-like lymphocytic infiltrate at the connective tissue were observed. The final diagnosis was OLP. The patient is now about 10 months follow-up, but no treatment was prescribed once the patient showed no symptoms.
