Resumo: A 51-year-old female patient, HIV+, was referred for evaluation of a white lesion on the buccal mucosa of the left side close to the labial commissure. The lesion had approximately 2 cm, presented clear limits and verrucous surface, and it was surrounded by a brownish halo, compatible with melanin pigmentation. An incisional biopsy was performed, and the histopathological examination showed squamous epithelium with hyperkeratosis, acanthosis and severe epithelial dysplasia. In addition, superficial cells presented koilocytosis suggestive of virus infection. Immunohistochemistry for p16 showed diffuse nuclear and cytoplasmic positivity in the surface epithelium. In situ hybridization for broad-spectrum HPV showed the presence of viral DNA in the nucleus of epithelial cells. All these features lead to the diagnosis of HPV-associated severe epithelial dysplasia. The patient was referred to the head and neck surgeon and total resection of the lesion was performed. The patient is being followed without clinical evidence of recurrence.
