Resumo: Female patient, 07 months old, exhibited swelling in the anterior region of the maxila with a fibrous consistency. A cone beam computed tomography was performed, which showed an extensive expansive and osteolytic lesion, with element 51 inside the lesion. Based on these findings, the clinical hypothesis was an adenomatoid odontogenic tumor. An incisional biopsy was performed, in which the intralesional fragments presented a whitish color and a diffuse blackish pigmentation. Microscopic examination revealed a neoplasm of ectodermal origin, consisting of a proliferative biphasic cell population, one composed of small round and hyperchromatic cells and the other cuboidal epithelioid cells that exhibited melanin granules, which were organized in islands, cords and alveolar structures permeated by a stroma densely collagenized and intensely cellularized. Based on these findings, the final diagnosis was melanotic neuroectodermal tumor of infancy, and the patient underwent surgical resection and is being followed up.
