Resumo: Fanconi anemia (FA) is a rare recessive genetic disorder characterized by congenital anomalies, bone marrow failure, and increased risk for malignancies, such as oral squamous cell carcinomas (OSCC). A 41-year-old male, submitted to hematopoietic stem cell transplantation (HSCT) for FA 18 years ago, presented a 2 cm exophytic and ulcerated painful lesion on the tongue with irregular borders. The lesion was close to a 2 month evolution white plaque on the floor of the mouth. He was absent for regular follow up for 3 years because of the COVID-19 pandemic. An incisional biopsy showed an invasive and moderately differentiated SCC. The patient was referred immediately to an oncology service for surgery. Frequently oral examinations are strongly recommended in patients with FA, particularly if they have undergone HSTC.
