Título: Clinicopathological analysis of Osteomielytis of the Jaws during a 51-year period

Resumo: Objective: To investigate the clinicopathologic characteristics of osteomyelitis of the jaws (OJ) in an Oral Pathology service. Study Design: A 51-year retrospective study was conducted. Data on clinical, radiographic, and histopathological features of cases diagnosed as OJ were collected and evaluated. Chi-square and Fisher´s Exact Test were performed to compare categorical features. Results: Among 17,991 cases, 75 (0.4%) were OJs. A predominance of female patients (89.3%) was observed, with a mean age at diagnosis of 42.68 years (±21.10). The majority were white (49.3%). Secondary chronic osteomyelitis (SCO) was the most common subtype (84%), followed by acute osteomyelitis (n=9, 12%). The mandible was the most affected site (93.3%), and odontogenic infection was the most frequent etiology (54.7%). Microscopic analysis showed a predominance of discrete mononuclear inflammatory infiltrate (38.7%); polymorphous inflammatory infiltrate was mostly absent (68.0%). The intensity of inflammatory infiltrate did not reveal significant associations with osteomyelitis subtype (p>0.05). Presence of bacterial colonies was associated with SCO (p=0.04). Conclusion: The clinical features observed were consistent with those reported in the literature. SCO is the most common OJ subtype. The classification and diagnosis of OJs are based on clinical and radiographic features, with histopathological analysis serving as an auxiliary tool for diagnosis.
