Resumo: Objective: To investigate the topical use of Brazilian organic propolis (BOP) as a therapeutic option for radiation-related oral acute toxicities. Study Design: Sixty oral and oropharyngeal cancer patients were included in this randomized, double-blind, controlled clinical trial. Patients were randomized to receive either aqueous suspension of a BOP or placebo throughout radiotherapy (RT). OM, pain, dysphagia, dysgeusia, and presence or absence of oral candidosis were assessed weekly. Protein levels of TNF-α and IL-1β from oral mucosa were assessed before and during RT by ELISA.Results: BOP group patients had a lower score of OM, dysphagia, dysgeusia, and most of them reported moderate pain. Fewer patients developed oral candidosis in the BOP group compared to those in the placebo group, and the number of episodes of oral candidiasis also was lower (p <0.05). Also, BOP group presented lower levels of IL-1β since the beginning of treatment when compared with placebo patients (p < 0.05) and a lower level of TNF-α at the end of treatment (p < 0.001). Conclusion: Topic use of BOP reduced TNF-α and IL-1β levels, oral candidiasis episodes, and seems to be a useful alternative option for prevention and treatment of the main acute oral toxicities of RT.
