Resumo: Dentigerous cysts are the most common developmental odontogenic cysts. They are classically characterized by the unilocular radiolucent areas associated with the crowns of unerupted teeth at the level of cementoenamel junction. The lesion is predominantly asymptomatic and most cases are detected during routine radiographic examinations. Nonsyndromic bilateral dentigerous cysts are rare. This study aimed to present a case of bilateral dentigerous cyst in a 9-year-old boy that presented an extensive swelling in the anterior maxila. Radiographic image analysis revealed large, well‑defined bilateral unilocular radiolucencies extending from the unerupted permanent maxillary premolar to central incisor causing displacement of the germs of the adjacent teeth. Aspiration and incisional biopsies were performed and two drains were inserted for decompression. After radiological follow up, enucleation and attachment bonding to the crown of the involved teeth was performed. The patient remais in orthodontic treatment and three year follow up reveals no signs of recurrence.
