Resumo: Ossifying fibroma (OF) is a benign fibro-osseous lesion characterized by the replacement of normal bone with fibrous tissue. A 20-year-old male patient reported painless swelling over the right mandible for the past 6 months. There was no history of previous trauma, and no associated paresthesia was reported. Intraoral examination revealed diffuse swelling over the right buccal vestibule involving teeth 46 and 47. The lesion appeared well defined on CT-Scan, with a sclerotic border, a central area of trabeculations radiating to the periphery, and multiple areas of calcification. The histopathological examination confirmed OF diagnosis after an incisional biopsy. The lesion was treated with intra-oral resection of the mandible, and five months later, reconstruction was performed using a microvascular fibula bone graft. A pre-bent titanium reconstruction plate was adapted using a 3D model as a template. In postoperative follow-up, the patient showed satisfactory healing with no clinical signs of recurrence.
