Resumo: AbstractOsteochondrolipoma is an extremely rare histological variant of lipoma. We hereby report a case of oral lipoma with areas of bone and cartilaginous differentiation. A 51-year-old male patient presented with a chief complaint of painless mouth swelling, with approximately 3 years of evolution. The lesion presented as a sessile nodule, with a smooth surface and pink color in the lower lip (3x3x2 cm). On palpation, the lesion was painless and soft in consistency. An excisional biopsy was performed. At the laboratory, it was noted that the lesion was floating in formalin. Microscopic examination showed a well-encapsulated lesion comprising lobules of mature univaculated adipocytes with central areas of bone and cartilaginous metaplasia. A final diagnosis of osteochondrolipoma was made. The treatment of this lesion consists of complete surgical excision, recurrences are not expected and the prognosis is similar to that of the other lipomas.
