Resumo: Necrotizing sialometaplasia is an uncommon, locally destructive inflammatory condition that most frequently affects the palatal minor salivary glands, although salivary glands at other sites can also be affected. Clinically, it is characterized by the presence of a painful, ulcerated swelling, but in early stages, the lesion might not be ulcerated. Although it is a benign, self-limited condition, care must be taken when diagnosing it, as it can mimic malignant processes, both clinically and histologically.In this report, we present two cases of necrotizing sialometaplasia affecting the posterior hard palate, clinically mimicking a malignant salivary gland tumor. Incisional biopsies were taken from both cases, showing characteristic necrosis of the salivary gland acini, while maintaining the main lobular architecture of the gland. Different degrees of squamous metaplasia of the salivary ducts, mucin extravasation, and foamy histocytes (CD68 positive) were also commonly observed features. Both cases were managed with topical corticosteroid therapy.
