Resumo: A 23-year-old male patient with general malaise and weight loss during the last three weeks complained of xerostomia and oral white patches in the oral cavity.Oral examination revealed macroglossia with lingual indentations. The pink color of the oral mucosa had changed to a white color. The lips, cheeks, and the dorsal, lateral borders, and ventral surface of the tongue appeared covered by a whitish membrane. The white paint was less pronounced in the vestibular upper and lower gingiva. Additionally, an odor of ammonia or urine was perceived in the patient´s breath.The white membrane was composed of the total thickness of the detached epithelium. The surface of the epithelium presented two to three rows of para-keratinized cells. The basal cells could be identified, and the spinous stratum showed polyhedral cells with clear cytoplasm and a central vesicular nucleus. No inflammatory cells were observed. Laboratory tests are presented.
