Resumo: EBV infects epithelial cells and B lymphocytes, integrating its proteins to those of the host cell, contributing to leukoplastic lesions. Female patient, 58, presenting a leukoerythroplastic lesion with irregular edges, measuring approximately 4cm in its largest diameter in anterior gingiva between elements 42 and 44, evolution of 3 years, negative Nikolski sign and diagnostic hypothesis of leukoplakia. An incisional biopsy was performed, with a diagnosis compatible with mucosal pemphigoid. On his return, ulceration associated with the leukoerythroplastic lesion on distal gingiva of 43 was noted, resulting in request of the IGA, IGM, IGG and EBV immunohistochemical panel with positivity for all markers and a report compatible with EBV-related desquamative mucositis. After result, a clinical attempt was made to use acyclovir cream in oralbase associated with improved oral hygiene. There was no positive evolution, opting for complete exeresis of the lesion due to neoplastic risk, with no signs of relapse.
