Resumo: A 78-year-old woman presented for diagnosis of symptomatic gingival lesions with a nine-month evolution time. Vesiculobullous lesions on the upper and lower gingiva, along with irregular and multiple ulcers in a pattern of desquamative gingivitis were found during intraoral examination. The patient also reported a persistent conjunctivitis affecting the right eye, which was irritated. Thus, the main diagnostic hypothesis was mucous membrane pemphigoid (MMP). An incisional biopsy was performed, and microscopically, the diagnosis of MMP was confirmed. The patient was referred to dermatologist and ophthalmologist for evaluation and treatment. Topical and systemic anti-inflammatory steroids were prescribed. However, she refused the therapy, reporting possible side effects of the medication. During the follow-up, the right eye developed symblepharon and its vision was compromised. Furthermore, in subsequent visits, it was observed that the disease had progressed and the patient´s left eye was also affected, similar to what occurred in the right eye.
