Resumo: A 48-year-old female patient had a painless intraosseous lesion involving the periapex of the maxillary right first molar. Intraoral examination revealed a slight swelling on the right buccal region. Tomographic images disclosed an unilocular, well-defined hypodense image measuring 13x12x11 mm. The pulp sensibility test of the tooth involved was negative, suggesting an inflammatory periapical lesion. An excisional biopsy was performed. Histopathological analysis exhibited a benign, non-encapsulated, solid lesion composed of cords and small sheets of odontogenic epithelium intermingled with nests of clear cells and amyloid-like material deposition. There was no evidence of calcified material. Both PAS and Congo-red staining were negative in the clear cells and the amyloid-like material, respectively. Neoplastic cells showed diffuse positivity for CK19, with focal positivity for CK7 and S100. Based on clinicopathological findings, the diagnosis of non-calcifying Langerhans cell-rich variant of calcifying epithelial odontogenic tumor was established. Support: CAPES; CNPq.
