Resumo: Giant cell granuloma is defined as a benign lesion, that affects the maxilla and mandible, locally aggressive resulting in soft tissue and bone envolvement during its course. We present a case affecting a native Brazilian, 4 year old girl, presenting a nodular ulcereted lesion, measuring approximately 1.5 cm in its largest diameter in the left mandible related to elements 36, 75, 74 and 73, with an evolution of less than 6 months, friable appearance, hemorrhagic. CT scan examination revealed apparent carious lesions on the referred teeth. Differential diagnosis comprised pyogenic granuloma peripheral giant cell lesion, sarcoma and hemangiopericytoma. Insicional biopsy was performed and the diagnosis of Giant cell granuloma with central involvement was made. Steroid sclerosing therapy was stablished for 2 sessions resulting in basilar bone formation after 45 days. The excisional biopsy was performed under general anesthesia. The patient was released after 2 months with no signs of relapse.
