Resumo: An 85-year-old woman sought a hospital emergency room due to a severe and generalized pain, with blisters and ulcerations on the skin, hemorrhagic crusts on the lips and ulcerations on the mucous membranes, preventing her from feeding. She reported the onset of these symptoms after a condition of pharyngotonsillitis treated with antibiotics a few weeks ago. The past medical history showed hypertension and hypothyroidism, both under medical control. The Stomatology team considered two diagnostic hypotheses: Erythema Multiforme and Paraneoplastic Pemphigus. Lip and skin biopsies and several laser therapy sessions were performed for bio stimulation and oral pain control. Histopathological examination (H&E and immunofluorescence) was conclusive of Pemphigus, with subtype to be clarified. Extensive clinical and laboratory medical research was carried out. The diagnosis of Paraneoplastic Pemphigus associated with monoclonal gammopathy of undetermined significance was established. The patient is doing well, undergoing treatment and the lesions are under control.
